Registration Procedure:

Please kindly fill in this form as thoroughly as possible. Payment must be done before the first class once classes have been booked on Okrabook system. 

Participation in classes is only possible after payment. We do not guarantee reservations of classes if payment is not made. Threshers swim club accepts bank transfers, online payments, cash or card payments. 

Squad swimmers: There will be a 10 Rial charge for squad swimmers per year, and additional 2 rial extra for every additional family swimmer, this is placed towards liability insurance and administration costs which can be done through our okrabook booking system in class packs

 

Class Dates:

Threshers swimming club will hold their classes during published school term dates, even if the class falls on a pubic holiday. I understand that the lesson fee pays for a class regardless of attendance.


Make up Lesson Policy:


  1. If classes are cancelled due to weather or pool issues, make up lessons will be granted for each class missed at one of our locations.
  2. In general Threshers Swimming club will allow 1 make up lesson regardless of the issue for any missed class per month. We may allow in case of an emergency, serious illness for longer than 1 week, or severe injury, once a doctor's letter is provided, more than one make up. However, we will not be able to give an extra make up lesson if the child has a slight fever or flue, if the issue arises that it is severe, we will asses the situation and act accordingly about possible make up lessons.
  3. Please be advised that make up lessons must be done during that same month, we shall not be able to extend make up lessons into a new month, unless given permission by the head coach.
  4. Family Bereavement: In the unfortunate event of a family death, we will provide a credit to the family for the missed days.
  5. Notification Requirement: Make up Lessons for cases under points 2) or 3) will only be approved if the coach is informed before 2pm on the day of the missed lesson.
  6. Lesson Reimbursement: We do not generally offer reimbursements for lessons that have already been paid for.
  7. Holiday leave: If you plan to take holiday leave please inform your head coach as early as possible. To be considered for rescheduling, the leave must be registered with the head coach before the monthly booking is done on the 25th of every month.

Photo's/ Video:

  • Threshers Swimming club is allowed to take pictures and videos of myself/my child solely to be used for training purposes
  • Threshers Swimming club usually takes photographs and video's to be used solely for promotional and informational purposes for the use for our company and electronic media. By my initials at the bottom of this page I give my consent for the participants image to be captured. No pictures/videos will be published of classes for women only (9 & above) without the consent of the parental guardian.

Authorization of Medical Care:

  • By my initials I agree that in case of an emergency, I authorize the lifeguards/staff of Threshers Swimming club to contact and/or secure medical attention for the participant. I am also aware that I must provide the participant/myself with up to date medical insurance coverage while participating in the training programs.

Accident Waiver and Release of Liability:


By enrolling myself and/or my child(ren) in the swim classes and related activities, I hereby voluntarily assume all associated risks. These risks include, but are not limited to, those arising from the negligence or carelessness of the participants, or affiliated entities; the use of defective or hazardous equipment or facilities owned, maintained, or controlled by third parties; or any other circumstances that may give rise to liability, including those occurring without fault.

I certify that the participant is physically fit, and has not been advised to not participate by a qualified medical professional. I certify that there are no health-related reasons or problems which preclude the participation in activity. In consideration of my application and permitting me/my child to participate in this swim class, I hereby take action for myself, my executors, administrators, heirs, next of kin, successors, and assigns as follows:


  1. I hereby agree to waive, release and discharge from any and all liability, including but not limited to, liability arising from negligence or fault of the entities or person released, for myself/my child's death, disability,, personal injury, property damage, property theft, or actions of any kind which may hereafter occur to me including my traveling to and from this place of activity, the following entities and persons: Hlava International LLC, Threshers Swimming Club, and/or their directors, officers, employees, volunteers, representatives, agents, and the activity holders, sponsors and volunteers.
  2. I further agree to defend, indemnify, hold harmless Hlava International LLC, Threshers Swimming Club, and promise not to sue the entities or persons stated in this paragraph form and all liabilities or claims made as a result of participation in this activity, whether caused by the negligence or release or otherwise.

I acknowledge that Hlava International LLC, Threshers Swimming Club and their directors, officers, volunteers, representatives, and agents are NOT responsible for the errors, omissions, acts or failures to act of any party or entity conducting a specific activity on their behalf.

The Accident Waiver and Release of Liability Form shall be construed broadly to provide a release and waiver to the maximum extent permissible under applicable law.


I CERTIFY THAT I HAVE READ THIS DOCUMENT AND FULLY UNDERSTAND ITS CONTENT. I AM AWARE THAT THIS IS A RELEASE OF LIABILITY AND A CONTRACT THAT I SIGN OF MY OWN FREE WILL.
